• Incomplete forms will not be processed and must be resubmitted.
• A separate form is required for each IATA number and needs to be submitted individually.
• Please print and complete the form and send to TktAuthority@etihad.ae attaching the supporting documents.
• You must include your Country name and IATA Number in the email subject line.
